They lead a coordinated team of geriatric-trained providers who ensure every patient receives attentive, professional, and fully integrated medical care.
Independent Living Communities
For residents who enjoy independence but prefer the peace of mind that comes with accessible medical care, CFGC delivers on-site primary health care directly to their community.
Our providers handle routine checkups, medication management, and preventive screenings without the hassle of travel — ensuring continuity even if a resident later transitions to assisted living.
Assisted Living Facilities
As medical needs increase, our involvement deepens.
We visit partner communities at least once per week, managing chronic conditions, providing on-demand evaluations, and collaborating closely with facility nursing staff. Our patients and their families know they can count on us for attentive, consistent support as care needs evolve.
Memory Care & Behavioral Health
Led by Deena Ganesh, DNP, our team provides specialized care for residents facing cognitive decline, dementia, or mental health challenges such as anxiety and depression.
Her dual certification in psychiatric and geriatric primary care allows us to address both the emotional and medical dimensions of aging, helping families and facility teams manage complex situations with compassion and confidence.
Urgent Care Integration
When a patient’s condition requires more advanced diagnostics or immediate treatment, we coordinate a priority visit to The Center for Urgent Care (CFUC) — avoiding unnecessary and stressful ER trips. At CFUC, patients have access to:
- ACR-certified CT and digital X-ray
- CLIA-certified laboratory testing
- IV medications and fluids
- Suturing and respiratory treatments
This partnership keeps care within one trusted system, reducing delays and improving outcomes.
Hospital Coordination & Direct Admissions
When hospital admission is truly needed, we handle direct admissions through our network of partner hospitalist teams — bypassing the emergency room entirely.
Because Dr. Avila and Dr. Bailey are on staff at local hospitals, we maintain access to hospital records and remain involved throughout the entire stay, ensuring seamless post-discharge follow-up and continuity of care.
Rehabilitation Hospitals
Some patients benefit more from rehabilitation hospital admission rather than acute-care hospitalization.
Our team evaluates each situation individually, coordinating with trusted local facilities to ensure patients receive specialized therapy and recovery care suited to their clinical needs.
Hospice Collaboration
When a patient’s journey reaches a stage where comfort becomes the priority, CFGC works hand-in-hand with hospice partners to ensure dignity, peace, and continued medical oversight.
