HOW TO REQUEST ACCESS
Please send an email to Medical Records requesting login access. Please include your full name and date of birth.
Notice: We will confirm the email matches what you provided us during registration for your privacy and security. Upon confirmation, you will receive an email with your login credentials.

Below are some simple “how-to” instructions to help you through it.

- Go to the Patient Portal.
- Enter the credentials you have received, and click the login button.
The Patient Portal Dashboard will open. Under the LATEST STATEMENT section on the right, you will see a blue button labeled Pay this bill. Click on it and follow the prompts to complete the payment.
A “Thank you for your payment” page will display once the payment has been received.
