A Different Kind of Primary Care

Understanding the CFPC Membership

The Center for Primary Care is a concierge membership practice designed to provide something increasingly rare in modern healthcare:
time, access, and continuity.

Annual Membership

$5,000 per year per member

(For the first 100 members)*

This model allows care to be delivered without the limitations of traditional insurance-based medicine.

Membership supports a comprehensive and proactive approach to adult primary care and includes:

We will perform a thorough, comprehensive health assessment and physical examination, with a focus on Preventive and Comprehensive Care.
We will make all of the necessary arrangements for any Referrals and/or Further Testing that might be required.
We will obtain one set of labs to be run in our own lab, to include the following:
- CBC (Complete Blood Cell Count)
- CMP (Comprehensive Metabolic Panel)
- U/A (Urine analysis)
- Lipid Panel
- HgbA1c (Glycolated Hemoglobin)

We will send out the following labs to Quest
- TSH (Thyroid Test)
- PSA (Prostatic Specific Antigen) – men
EKG
CXR (Chest X-Ray)
Availability for same-day appointments
Unlimited telemedicine calls as needed
Direct Access 24/7 from anywhere in the world via phone, text, or email for any emergency

*Note: Fee subject to change after our first 100 members.

Financial

& Insurance Information

The Center for Primary Care operates on a membership-based model and does not bill commercial insurance. All services are considered self-pay. Upon request, our office may submit claims to your insurance carrier for possible reimbursement.

For patients with Traditional Medicare, we participate as a Medicare provider and will submit claims directly. Patients remain responsible for any applicable deductibles, copayments, or coinsurance.

This structure allows for more comprehensive, personalized care without the limitations of traditional insurance models.

How to Access Care

EMERGENCIES
For life-threatening emergencies, call 911 or go to the nearest emergency room immediately, and notify us at 941-259-6450 as soon as you can.

URGENT MEDICAL CONCERNS (8 am-8 pm / 7 days a week)
For acute issues such as chest pain, shortness of breath, injury, or sudden illness, patients may go directly to The Center for Urgent Care. If possible, please notify our team at 941-259-6450 so we can coordinate care.

ROUTINE & PREVENTIVE CARE
Appointments, follow-ups, and medication refills can be scheduled by calling or texting our office at 941-259-6450. Telehealth visits are available when appropriate.
